HOW TO REGISTER FOR THE SUNDAY EVENING COURSE BEING HELD IN HOWARD COUNTY AT COVENANT PARK
Registration for the Sunday Evening Spring 2012 Entry Level Course is not open at this time. The course is planned to start in February 2012 and will run six (6) consecutive Sunday Nights from 6pm to 9pm. Students who want to register will need to follow these registration steps.
1- Click the link here [REGISTER] to be redirected to the Online Registration Form. Complete this Online Registration Form and then click "Register Now" button at the bottom of the form. You will be redirected back to this page.
2- Complete the Online USSF Registration Form (as described below).
3- Send payment of the course fee of Twenty-Five Dollars ($25) payable in cash or check to Craig Proffen to:

The first 40 registrations received will be guaranteed a spot in the Spring Sunday Night 2012 Entry Level Course.
A confirming email announcement will be sent to everyone accepted to the course once their registration is completed.
The course is expected to be held at the new Administrative Office of the Soccer Association of Columbia/Howard County. The address is 4560 Centennial Lane just off Route 108 at the sign: Northrop Fields at Covenant Park.
STUDENTS WITH ANY SPECIAL NEEDS MUST MAKE THOSE REQUESTS NO LATER THAN 30 DAYS PRIOR TO THE START DATE OF THE COURSE.

HOW TO REGISTER FOR THE SPRING 2012 WEEKEND COURSE BEING HELD IN HOWARD COUNTY AT JOHN'S HOPKINS APPLIED PHYSICS LAB
Registration for the Spring 2012 Weekend Entry Level Course is NOW OPEN for registrations.
1- Click the link here [REGISTER] to be redirected to the Online Registration Form. Complete this Online Registration Form and then click "Register Now" button at the bottom of the form. You will be redirected back to this page.
2- Complete the Online USSF Registration Form (as described below).
3- Send payment of the course fee of Twenty-Five Dollars ($25) once registration opens.

HOW TO REGISTER FOR A RECERT CLINIC & WHAT DO YOU BRING
Registration for Monthly In-Service Clinic or Annual One Day Recert is the same. Referees who want to register will need to complete the Online USSF Registration Form (as described below) and pay the Annual USSF Licensing Fee (based on their referee grade) either Online at USSF or by Check at the Clinic Payable to MSR (Maryland Soccer Referees). If payment is made Online to USSF proof of that "Online Payment" MUST be provided to the Clinic Instructor when taking the recert exam. Referees MUST also reserve a seat by emailing the Lead Instructor and provide the following information in an advance email (to Craig Proffen for Howard County Clinics) with "Recert Clinic" as the subject line:
1- Full Name 2- Email Address(es) 3- Clinic Location & Date Wish to Attend 4- Contact Phone Number(s)

| USSF Referee Registration Form |
Completion of the USSF Registration Form is now mandatory for all students who want to become a USSF Licensed Referee and enroll in an Entry Level Course. The form can be found here at this web site and students MUST follow these instructions EXACTLY as they are given here:
1- Click here or navigate the web site to "Online Forms" then to "2011 USSF Referee Form".
2- Complete online (fill in the form) all applicable areas carefully (form can not be downloaded).
3- Once you have filled in (completed) the form online, PRINT two (2) copies. DO NOT print the form first and fill it in by hand. This is NOT acceptable!
4- Do NOT sign and date the form at this time. Students will do this in class.
5- Students enrolling in an Entry Level Course MUST mail one (1) copy to Craig Proffen (as addressed above) with their Registration Fee of $25 and bring the second copy to the course with them.
