| This registration form is to be completed by all students taking an Entry Level Referee Course. This form is also to be completed by referees who are recert'ing. Everyone completing this form must open the PDF Document, fill in the requested information, check for accuracy, then print the completed form. Do NOT print the form and then hand write in the information. Currently licensed referees will need to hand write their USSF ID # and game count. Signing and dating the form should be done on the date of the written exam. |
